RESUMO

In this work, a dual-pronged approach- (i) novel thin-film nanocomposite polyether sulfone (PES) membrane with MIL-101 (Fe) and (ii) 3D printed spacers were explored to enhance water recovery by forward osmosis. The concentration of PES, pore former, draw solution, and MIL-101(Fe) was optimised for maximum pure water flux (PWF) and minimum specific reverse solute flux (SRSF). The best membrane exhibited a PWF of 7.52 Lm-2 h-1 and an SRSF of 0.33 ± 0.03 gL-1 using 1.5 M NaCl and DI water feed. The M22 membrane with the diamond-type spacer demonstrated a PWF of 2.53 Lm-2 h-1 and SRSF of 0.75 gL-1 for emulsified oily wastewater feed. The novel spacer design imparted significant turbulence to the feed flow and a lower foulant resistance of 1.3 m-1 as compared to the ladder type (1.5 m-1) or commercial spacer (1.7 m-1). This arrangement could recover 19% pure water within 12 h of operation (98% oil rejection) with a ∼ 94% flux recovery after hydraulic wash.

BACKGROUND: Mycobacterium tuberculosis causes tuberculosis (TB), an infectious lung disease. There is mounting evidence linking low lipid levels to a variety of human diseases, including TB. Cholesterol, mainly due to its involvement in heart disease, gets more attention in recent years. The objectives of the study were to look into the link that connects hypolipidemia to the existence of pulmonary/extrapulmonary TB; we have tried to find the link in relation to patients who have been recently diagnosed with TB as well as in those who are having TB in the long term. MATERIALS AND METHODS: An observational study was performed on TB patients attending respiratory medicine at the Saveetha Medical College and Hospital, Chennai, Tamil Nadu, India, from February 2021 to January 2022, and their lipid levels were tested from patients with consent and correlated. Student's t-test was applied to the obtained data. To convey quantitative data, measurements such as mean along with standard deviation were applied, and a p-value of 0.05 was considered statistically significant. RESULTS: This research included 80 subjects, 40 of whom were diagnosed with TB, and the rest (40 controls) were deemed healthy. The age group with the highest low lipid levels in pulmonary TB was 40-50 years. A chi-square test of association was conducted; this test revealed that the fraction of TB patients having lower than normal levels of total cholesterol (p = 0.0001), triglyceride level (p = 0.006), high-density lipoprotein (p = 0.009), low-density lipoprotein (p = 0.006), and body mass index (p = 0.000) was statistically significantly higher in contrast to the control group. Thus, there was a significant correlation between a higher prevalence of hypolipidemia in patients with pulmonary tuberculosis (PTB) and normal healthy individuals. CONCLUSIONS: We observed a strong relationship between hypolipidemia and TB, indicating that patients with low lipid levels tend to have severe inflammation as compared to patients with normal lipid levels.

Background and purpose The patients after amputation undergo a sudden transformation in their quality of life. In India, amputation done at the appropriate time is a rare phenomenon because usually, the patients present themselves at the later stages. The surgeons, however, while performing amputation surgeries, primarily consider saving the life of a patient under adverse conditions when patients report to them very late that the surgeries are carried out urgently. Assessing the quality of life (QOL) and the various sociodemographic factors affecting the QOL paves the way for future rehabilitation programs. Aims and objectives To evaluate the quality of life of subjects with unilateral lower limb amputation among the North Indian population. Materials and methods This cross-sectional study was conducted in the tertiary rehabilitation center. A total of 106 Subjects were recruited. Informed consent was taken. WHOQOL-BREF contains 26 items covering four important aspects of QOL. The WHOQOL-BREF self-administered free questionnaire was used as a data collection tool, and the Hindi version downloaded from the WHO website was also used for those who can't understand English. Results The range of the physical domain, psychological domain, social domain, and environmental domain were 0 and 100. The mean score of different QOL transformed domain scores (on a scale of 100) were 47.91±20.12, 57.37±20.46, 59.36±25.32 and 51.50±21.96, respectively. Trauma was the leading cause of amputation, followed by diabetes mellitus, cancer, peripheral vascular disease, and other causes. Transtibial amputees were more in number compared to transfemoral. The percentage of male and female amputees was 78.30%, and 21.70%, respectively. Conclusion The physical domain was the most affected domain, followed by the psychological, social, and environmental domains. A delay in the prosthesis fitment aggravates the physical burden of amputees. Early prosthesis & psychological counseling will improve the QOL significantly.

OBJECTIVES: To evaluate the autonomic function in specific learning disorder (SLD) and comorbid SLD attention-deficit hyperactivity disorder (SLD-ADHD). METHODS: A cross-sectional study was conducted in a tertiary care hospital with 20 adolescent subjects each of confirmed SLD, SLD-ADHD, and healthy control (mean age 15.32 y). Heart-rate variability and autonomic-function tests were carried out using standard protocols. RESULTS: Heart-rate variability parameters, viz., mean RR interval, number of RR intervals which differ by ≥ 50 ms (NN50), percentage NN50, standard deviation of differences between adjacent RR intervals, root square of mean of the sum of the squares of differences between adjacent RR intervals, coefficient of variance and absolute power of high-frequency band (HF) recorded apparently lower levels in SLD and SLD-ADHD as compared to healthy control indicating lower parasympathetic tone. Whereas, higher absolute power of low- frequency band (LF) in SLD and SLD-ADHD than healthy control indicated enhanced sympathetic activity. Higher LF/HF and lower SD1/SD2 ratios in SLD and SLD-ADHD than healthy control indicated higher sympathetic tone over parasympathetic tone. Values of autonomic-function tests such as E:I ratio, change in heart rate during deep-breathing test, 30:15 ratio, and Valsalva ratio showed a decrease in SLD and SLD-ADHD as compared to healthy control implying reduction in parasympathetic reactivity. Increased values for rise in diastolic blood pressure in the isometric handgrip test and cold pressor test recorded in SLD as compared to healthy control, revealed the increased sympathetic reactivity. CONCLUSION: Overall, results of heart-rate variability and autonomic-function tests imply dysregulation of sympathetic and parasympathetic activities with sympathetic dominance in SLD and SLD-ADHD.

RESUMO

The Ubiquitin-Protease system (UPS) is a major destruction system responsible for eliminating dysfunctional/misfolded proteins, thus acting as a pivotal regulator of protein homeostasis in eukaryotic cells. In this review, the UPS system and its various functions in the cell and their detailed impact such as cell cycle control, DNA damage response, apoptosis, and cellular stress regulations have been elucidated with a focus on the central nervous system. Since the Ubiquitin-Protease pathway(UPP) plays a prominent role in the sculpting of the CNS cells and their maintenance, it is naturally deeply involved in many malignancies that develop due to dysregulation of the UPS. Understanding the major disruptive players of the UPS in the development of these malignancies, for example, insoluble protein aggregates or inclusion bodies deposits due to malfunctioning of the UPS has paved the pathway for the development of new therapeutics. Here, the de-regulation of the UPS at various checkpoints in CNS malignancies has been detailed, thus facilitating an easy comprehension of the different targets that remain to be explored yet. The present therapeutic advancements in the field of CNS malignancies management through UPS targeting have also been included thus broadening the scope of drug development. Thus, this review while shedding sufficient light on the details of the UPS system and its connection to CNS malignancies, also opens new avenues for therapeutic advancements in the form of novel targetable UPP proteins and their interactions.

Corona virus disease was first reported in December 2019 in Wuhan, China and is spreading across the world in an alarming fashion. To contain the spread, it is very important to identify the subtle/not readily apparent symptoms of COVID-19 at the earliest. The aim of the study is to determine the incidence duration progress of sino-nasal symptoms in COV 2 positive patients using SNAQ scoring. Patients who tested positive for SARS-COV 2 by RT-PCR and admitted in our hospital under category A (n = 382) were included in the study. A detailed history was collected from all the patients and sino-nasal assessment questionnaire (SNAQ) was provided to the patient with complaints of sinonasal symptoms and they were asked to fill the forms on day 3, 7 and 14. To identify the characteristics of sinonasal symptoms in COVID-19 patients with a history of smoking, smoking history was also collected in detail and patients were classified based on Brinkman's index into mild, moderate and severe smokers. In this study, the incidence of sinonasal symptoms was 24%. Average SNAQ scoring on day 3 was 30.09 and on day 7 was 12.9 and day 14 is 3.8. There was a decline in score on day 7 compared to day 3 indicating symptoms decrease by day7. Average SNAQ scoring in non-smokers and mild and moderate smokers was 20.18, 34.11, 57.5 respectively. The SNAQ scoring in smokers was more than that of non-smokers and was also persistent for a longer duration compared to non-smokers. Sino-nasal symptoms catch our eye because it is an important route for transmission. Viral shedding from sinonasal tract may be an important source for transmission. History and degree of smoking should also be considered while dividing COVID-19 patients into categories.

We report a novel class of chitosan (CS)-based hybrid semi-interpenetrating nanogel catalyst by the reductant-free green synthesis of gold nanoparticles (Au NPs) within the CS-poly(methacrylic acid) (Au@CS-PMAA) polymer network. The covalently cross-linked Au@CS-PMAA nanogel with CS chains semi-interpenetrating (semi-IPN) in the PMAA cross-linked network show superior colloidal stability and improved catalytic activity. The synthesized Au@CS-PMAA semi-IPN nanogel was applied as a superior modifier to create a new and sorely selective electrochemical sensing platform for the determination of antileukemic drug 5-fluorouracil (5-FU). The existence of -OH and -COOH groups in the CS and PMAA offers numerous binding sites for 5-FU. This assures the incorporation of Au NPs which affords a larger surface area and ameliorate electrical conductivity. Besides, the synergic effect amid both compounds improves the detection of 5-FU. Under optimal conditions, the constructed sensor offered a good performance in determination of 5-FU with a wide linear range from 0.1-497 µM and low detection limits of 0.03 µM, respectively. Further, this study proved that the fabricated sensor could be useful for analysis of 5-FU in actual human blood serum samples.

Decades have passed since septoplasty was first introduced for the management of the nasal airway. Various modifications of the technique have been made to deal with the pathology of the nasal septum. The authors have used the nasal endoscope successfully for the ultraconservative management of the deviated nasal septum and turbinoplasty [inferolateral partial resection of the turbinate/s]. The present study is an update series of the previously published preliminary series, and comprises of 480 cases of deviated nasal septum who underwent endoscope aided septo-turbinoplasty [EAS] to improve the nasal airway. The subjective assessment of the efficacy of this procedure was done by visual analogue scoring system and the objective assessment was done by endoscopy. This study demonstrates significant improvement in relieving nasal obstruction and the contact areas. The authors advocate a combined approach-an endoscopic approach for inaccessible posterior deviation and a conservative traditional technique for accessible anterior segment [caudal septum].
